Proof of valid health/accidental insurance with full international coverage in Czech Republic. Health insurance card is not sufficient. A letter of confirmation of health insurance coverage issued and approved by the university or an insurance company is required. The letter must include the following information: - Insurance coverage is valid throughout Czech Republic. - Insurance coverage is guaranteed for the duration of stay in Czech Republic. - The coverage is at least 30,000 Euros (approximately $45,000.00 US). - The coverage includes medical treatment (injury, illness, emergency medical evacuation etc.), emergency reunion and repatriation of remains. - The name and contact number of the appropriate assistance service company must be included in the confirmation letter. |

A notarized copy of a credit or debit card (Visa, Mastercard...) in your name and with a validity for at least the duration of your stay in the Czech Republic, you may cover the credit card number before you make a copy of it, it is not necessary to send a copy of both sides of the card, the copy of the front side is sufficient

- a letter from a host or sponsor (parents must also submit current bank account statement) confirming support of the applicant during his or her stay in the Czech Republic - must be an original document with a notarized signature and presented with an official translation in the Czech language.
* The minimum amount of financial resources acceptable is: approx. 1500 USD + 120 USD/per each month of stay. |

Document confirming the purpose of stay
An original or a copy notarized by a Czech notary of a document certifying acceptance into a study program in the Czech Republic, issued by a Czech university or school. This document must be in the Czech language.

- In addition, if you have lived in any country for 6 months or more in the previous three years, you are required to submit the Criminal History Background issued by that country.
- For non US nationals only. You will need the Criminal History Record from the country of your nationality. If you have lived in the country only up to age 15 you must write a statement that you have not been involved in any crimes; above the age of 15 you must present the Criminal History Record."

Visas are usually processed in 70 working days. Rush and Emergency service may be available for additional fees. Please visit our Consular Fees page for details. Rush and Emergency service is defined as any attempt made by Travisa to retrieve the visa in less time than the amount required by the embassy to process the visa. |
| In order to ensure that visa applications for most embassies/consulates are submitted on the day that we receive them, they should be shipped to us via FedEx Priority Overnight (not First Overnight or Standard Overnight). |
